Zostavax billing
Zostavax is billed through a patient's insurance if they have commercial insurance and do not have
Medicare.  It is not covered by Medicare Part B, as other services would be in our office.  It is covered
through Medicare Part D which contracts with pharmacies.  There are some pharmacies which will bill
through Part D insurance, only charging your co-pay when they give the vaccine.  We charge the full price
for the vaccine when we give it to patients with Medicare.  We then give the bill so that the patient can
forward the bill to their Part D insurance to get reimbursed in part or in full, depending on your insurance
coverage.

The cash price is $231.  There is a 20% discount when patients pay the full amount on the day of service.
Sutter Independent Physicians HMO insurance
Zostavax is covered by the Sutter Independent Physicians HMO.  
This includes Secure Horizens and Seniority Plus, among others.
